ELIGIBILITY CRITERIA

Individuals may be certified to receive Home Delivered Meals based on the following criteria:

1. HOMEBOUND - the person cannot leave his/her house under normal circumstances due to illness and/or incapacitating disability and is unable to participate in the congregate meal program AND
2. LIVES ALONE and is physically or emotionally unable to obtain food and prepare meals and there is not one else available to prepare the meals.

Application for Home Delivered Meals is taken by referral from individuals or medical persons that feel the individual is in need of a home delivered meal.  Once we receive the referral, our office dispatches a coordinator for an intitial assessment and eligibility for service.  Additional assessments are conducted every six (6) months. 

Contributions are accepted to help offset the cost of these meals.

Unfortunately, when the demand for home delivered meals exceeds our budget, we make every attempt to service the applicant.  In cases where services are not immediately available, we will place the applicant on a waiting list for service.
