Another aspect of the Medicaid Waiver Program.

We cover these 8 counties.

We provide Case Management services through the Title XIX Aged and Disabled Medicaid Waiver Program. Elderly and/or disabled individuals, who qualify according to state guidelines, are assisted in getting services required to maintain independence in their homes. Case Management Services are provided in the following counties: Raleigh, Fayette, Wyoming, Summers, Nicholas, Mercer, McDowell and Greenbrier.
